AUTHORIZATION FOR A TRAVELING MINOR

Requirements for Minors

 

  • 2 photocopies of the application form. All minors that are 13 years of age at the time of application and not yet 18 years old must sign the application form and send a picture ID with notarized signature. If the minor does not have a valid picture ID, their signature can be endorsed before a Notary Public using the "Signature Form" with a passport size picture attached to it. The parents can print and fill out the Parent's Authorization form found below.

  • 2 photocopies of Signature Form (for all minors 13 years old and older).

  • The Application must always be signed by the parents. If the child is not yet 13 years of age, both parents must sign an application.

  • 2 photocopies of old passport (only first and second page).

  • 2 photocopies of Birth Certificate, if applying for the first time.

  • 2 copies of Parents Authorization Form, signed by both parents, who should sign in identical fashion to their Cedula.

  • 2 photocopies of Father’s and Mother’s Panamanian Cedula or passport.

  • 4 passport pictures and payment (see requirements above for passport renewal).
